Life Plan

PlanNET is a not-for-profit, non-government organisation founded, directed, and monitored by families for families that have a member with a disability.

PlanNET can facilitate ‘whole of life’ plans which are informed by the person and their family, focuses on their dreams for the future, works out what they don’t want, and arrives at a vision of what they do want. How that vision is achieved and who will contribute to its achievement is a vital part of the planning process. It is an active document that reflects how a person wants to live and their goals for the future.

This plan:


  • Puts on paper what has previously been unwritten
  • Identifies information that others might need to know
  • Highlights the person and not the disability
  • Focuses on the person’s strengths
  • Identifies dreams (and perhaps nightmares)
  • Outlines the goals toward achieving those dreams, and the steps to be taken
  • Helps the family to differentiate between their own ideas and their family member’s ideas about what is important
  • Identifies current resources, and those necessary to support the person to achieve their dreams.

This is a holistic plan, far more extensive than one’s NDIS plan, but provides the basis for it and/or other care plans.   

PlanNET can work with families to develop their unique plan, either on a one-to-one basis or as part of a group workshop. This may be funded from one’s NDIS plan budget.
